Important Health and Safety Notice Regarding COVID-19
By now you know that our office is a cash based practice. That means we expect payment up front for online bookings and/or at the time of service, unless you have legal representation due to a personal injury related history. If you have insurance, as a courtesy we will assist you by providing you with an invoice to submit to your insurance company. You will then be reimbursed directly from your carrier. Be aware that the insurance contract is between you and your insurance company and that it is solely your responsibility to make payment for services rendered at our office. If you need help verifying your insurance’s level of participation, as a courtesy we will gladly assist you by giving you a list of questions to ask your insurance representative. Please remember that verification is a summary of benefits and is not a guarantee of payment.
DISCLAIMER:
You can ask for a refund at any time. We will prorate your cash care plan and refund you the balance, although we will count your initial visit at the original $80 price.
There is no expiration date before which you must use your purchased visits. They do not expire. Ever.
